  3. Relate strategies for implementing self-management support into clinical practice. 

Challenge 1: Multiple Chronic Conditions: A Patient Perspective

To complete this challenge, consider Mae’s situation and compose a reflection based on the following prompts:

  1. Does anyone you know have multiple chronic conditions? What is it like for them? Does the video represent their “day in the life?”
  2. I-f you were a clinician working with Mae, what could you do to better her “day?”

Challenge 2: Introduction to Self-Management Support

Self-management support is the help given to people with chronic conditions that enables them to manage their health on a day-to-day basis. Self-management support can help and inspire people to learn more about their conditions and to take an active role in their health care. Work through the Self-Management Support elearning module to learn more.

To complete this challenge, compose a reflection based on your experience with self-management support either as a patient or a clinician.


Challenge 3: Implementing Self-Management Support

Incorporating self-management support into the daily routine of clinical practice can be challenging, but there are specific strategies to assist clinicians and many helpful programs and resources available, such as AHRQ’s Self-Management Support Resource Library

Work through the Implementing Self-Management Support elearning module to learn some of the strategies for implementation.

To complete this challenge you will develop a personal action plan.

Review the first two steps in the Effective Action Plan Discussions section of the module: 1) Have the patient choose, and 2) Be specific. After reviewing the content:

  • Develop a personal action plan. You are the patient for this activity.
    • Select a goal to change or improve your own health.
    • State your goal clearly.
    • Indicate the domain that you want to work on (Have the patient choose).
    • List the specific ways in which you will implement your plan (Be specific).
